Founder
Dr. Travis Livori is the founder and head Osteopath at Forme. He completed his study in a Bachelor of Health Science/Bachelor of Applied Science (Osteopathy) at RMIT University in 2017. He began his practice of Osteopathy in 2018 working at Moonee Valley Osteopathy and Keilor Health Centre, before opening Forme in 2021.
Travis approaches his work with care and diligence and prides himself on supporting people with their health concerns. He takes the time to understand the needs of his patients and works with them to achieve their recovery and lifestyle goals. He uses a broad range of therapy techniques to tailor his treatment to the individual concerns of his patients.
Travis enjoys playing a round of golf, is a proud member of the Collingwood cheer squad and spends his free time relaxing with family and friends.
Travis is registered with AHPRA (Australian Health Practitioner Regulation Agency) and is a member of Osteopathy Australia.

Health Science (Osteopathy). During his studies, Ethan worked with his local PEGS football club and within the NAB league as a sports trainer. He is qualified in Dry Needling, Myofascial cupping and has undertaken training with our Forme head Osteopath, Dr. Travis Livori throughout the duration of 2022. Ethan’s work is driven by his desire to create positive change in people’s lives. He adopts a variety of manual techniques during his treatments and takes time to empower and educate his patients on how to manage their pain and development outside of the consult room. Ethan loves anything outdoors and enjoys spending his time outside of work camping, fishing and watching a game of footy. Ethan is registered with AHPRA (Australian Health Practitioner Regulation Agency) and is a member of Osteopathy Australia.
Dr. Eden Cassar is a graduate Osteopath of RMIT University with a double degree in Health Science and Applied Science (Osteopathy). Throughout her studies Eden worked with her local Keilor Football Club as a sports trainer where she managed sporting injuries through manual therapy techniques and taping modalities. She is qualified in Dry Needling, Myofascial cupping and has undertaken training with our Forme head Osteopath, Dr. Travis Livori throughout the duration of her final years of study.
Eden’s passion for Osteopathy developed at a young age after a series of sports related injuries sparked a passion for assisting others with pain management. Empowering people with the knowledge to understand their injuries and independently create meaningful change is at the heart of her work.
She has been a part of the team at Forme since opening in 2021, where she worked assisting with the clinics front of house. As a practicing Osteopath she adopts a patient-centred approach to treatment and her consultations incorporate an array of manual therapy techniques and tailored rehabilitation planning to optimise pain management and wellbeing.
Outside of work, Eden enjoys staying active through playing sport, going to the gym, travelling with family and friends and spending time with her much loved dog, Remi.
