No, you do not need a referral to book in with us! You can simply contact our reception on (03) 9356 3140 or book online here to see one of our Osteopaths.
Upon arrival, you will be asked to fill out a new patient form before commencing your consultation. Your Osteopath will conduct a case history, an examination and provide you with a diagnosis before discussing treatment options. Treatment will begin in the second half of your appointment. Your Osteopath may refer you to a GP for further investigations or your local radiology clinic for diagnostic imaging.
Please allow one hour for your initial consultation at Forme, and 30 minutes for subsequent standard consultations.
Being pregnant is a wonderful and exciting time in life, however it can also cause some discomfort to your body. We treat women through all stages of pregnancy, providing manual therapy, exercise direction and lifestyle advice. We use a pregnancy pillow for the duration of your treatment to allow you to lie comfortably on your tummy if you wish.
Yes – we are a family focused clinic who treat patients through all stages of life, including babies and children.
If you have an Enhanced Primary Care (EPC) plan or a Chronic Disease Management (CDM) plan from your GP, a portion of your appointment will be covered by Medicare, and you will only be required to pay the gap. For further information on these care programs, we encourage you to speak with your GP.
Forme Osteopathy accepts all Private Health Insurance providers. If you are covered, we offer instant rebates after your appointment via our HICAPs machine, and you will pay the remaining gap payment. If you do not have your card or details on you that day, not to worry – you can pay the fee in full, and our friendly reception staff will instantly email you a copy of the invoice for self-online claiming.
Yes, here at Forme we treat all patients. If you are claiming Osteopathic care through WorkSafe, TAC or DVA we require accounts to be settled on the day of treatment. Our helpful reception staff will provide you with an invoice via email for you to submit to your relevant third-party.
Osteopaths are licensed physicians and Allied Health providers. In Australia, an Osteopath must complete a minimum of 5 years full-time university training, made up of a three-year bachelor’s degree and a two-year master’s degree, or a double bachelor’s degree. Osteopaths are required to be registered with the Osteopathy Board of Australia to practise, which is governed by the Australian Health Practitioner Regulation Agency (AHPRA).
Yes, please bring along any radiographs, diagnostic images and results to your appointment as any medical records will be helpful in providing as much information for your physical assessment as possible. After undertaking your physical examination and diagnostic testing in your appointment, your Osteopath may decide that it is appropriate to request additional investigations such as X-Ray, MRI, CT scans or blood tests.
You do not need to bring anything else to your appointment! We are a family and pram friendly clinic and encourage parents and carers to bring children and babies along.
You may wear any form of clothing to your appointment. For the purpose of examination and treatment, your Osteopath may ask if you are comfortable removing relevant items of clothing. We provide fresh towels to cover yourself during your appointment to ensure you feel comfortable and safe.
Our clinic is conveniently located in Centreway, Keilor East. There are plenty of one and three hour parking options available.
Yes, upon completion of your appointment please ask our friendly reception staff to provide a certificate of attendance. This will be sent through via email.
